Step 4: Click the link for the Provincial TD1 form for the province you reside in

Step 5: Complete the form by filling in the fields on your computer

Step 6: Save the completed form to your computer

Step 7: Return to this page and upload your completed forms to the file upload box below. Click “Submit” once you have uploaded both forms.
